These requests can be accomplished in 2 ways:

1. Call the office between 8:30am and 4:30pm and talk with a staff member. If you get a recording, then leave a message. Our staff checks messages hourly and will return your call or refill your prescription as soon as possible. Make sure you leave your full name, birth date, return phone number, pharmacy number, medication or test your requesting, dose, and frequency.

2. All of these, plus other, requests can be made online through MDHub (www.mdhub.com). This is an online communication service allowing patients to contact their physician's office through the internet. Just enter our phone number on the web page and then complete the appropriate form. MDHub then will fax a copy of your request to our office, and we get this immediately. If your request can be handled without a call back, we will take care of it for you. If a call back is needed, then we'll call back within 2 hours.
